Birth, Death, & Marriage Certificates
Birth Records Request Requirements 
Connecticut birth records that are less than 100 years old are not open to the general public***.  To be eligible to receive a copy of a birth certificate you must be able to document that you are related to the registrant in one of the following ways:

• You are the registrant, and you are 18 years of age or older with valid picture ID.
• You are the parent, guardian, grandparent*, or spouse** of the registrant with valid picture ID.
• You are the child* or grandchild* of the registrant, and you are 18 years of age or older with valid picture ID.

*You must provide a long form birth certificate to show your relationship to the registrant and your valid picture ID (ie: a grandparent would need to provide a birth certificate showing themselves as parent of their child).
** You must provide a marriage certificate in addition to your valid picture ID.
*** For Genealogical Research (including Birth records that are over 100 years old), we only process requests that include the fees for said certificates. They are $20.00 each and said money order should be made payable to the City Clerk (no personal checks accepted). Any fees that are sent in will either be used towards the certificate(s) that are found or if no records are found the fees will be used as payment for the search that we perform.

Description of Birth Certificates
Long Form (Required for a passport) - Lists parents and all the information. Cost: $20 (Additional $7 for use of Credit Card over phone)
Birth Card – Lists name and date of birth. Cost: $15 (Additional $7 for use of Credit Card over phone)

Four options for obtaining a vital record (birth, death or marriage)
Please select the option you want for specific instructions. On the forms, "Date of Vital" refers to
Date of Birth, Date of Marriage, or Date of Death.  This is how vital records are recorded.


Option 1: Appear in person in the City Clerk's office located in City Hall Room 215 with Valid Current Photo Identification. Payment is accepted in cash, debit, or credit. No personal checks accepted. Hours of operation are 8:30 am – 4:30 pm M-F.

Option 2: By Mail payment by money order (no personal checks) payable to “City of Norwich” with regular return mail service. Please complete the Regular Mail Order Form and attach the appropriate identification (refer “Birth Record Request Requirements” above) and enclose a self addressed stamped envelope. Mail to: Norwich City Clerk, 100 Broadway, Norwich, CT 06360. The day we receive your request is the day it will be returned. 

Option 3:  Email or fax request, payment on a debit or credit card, with regular return mail service. Please complete the Credit Card US Mail Order Form and include a phone number so we can reach you and fax* (860) 823-3790 or email it to: Roseanne Muscarella, Assistant City Clerk and attach the appropriate identification (refer “Birth Record Request Requirements” above).  (If you have trouble saving the document you may use PrimoPDF to make the document a savable document.  A free download can be found here.) Upon receipt someone will call you for your credit card information (provided you reside in the continental United States.  All others please call us 860-823-3733).

Option 4: Email request, payment on a debit or credit card, with FEDEX** return service. Please complete the Fedex Vital Request Form and include a phone number so we can reach you and fax* (860) 823-3790 email it to: Roseanne Muscarella, Assistant City Clerk and attach the appropriate identification (refer “Birth Record Request Requirements” above). *If you are faxing your request please enlarge and lighten your ID so that we can read
everything on it.
** This must be a street address.  FedEx does not deliver to a Post Office Box.

If you do not have photo identification, a copy of two of the following items can be substituted:

  • Signed Social Security card
  • Written verification of identity from employer on employer’s letterhead
  • Automobile registration
  • Copy of utility bill showing name and address
  • Checking account deposit slip stating name and address
